In order to complete your department's online renewal, you will need a list of the names, email addresses, and sections (e.g. PRODS, UMEDS) of any eligible members in your department.
At the end of this form, you will have the option to pay via credit card or to mail a check. If paying via credit card, please have your credit card information at hand when you complete this form.
This form is for regular (fully accredited, i.e. medical schools) institutions.
